We will ask you to provide some basic information to help us do that and want to make this process as hassle-free as possible.     New Patients

    At Berkeley Periodontics, we offer comprehensive periodontal services to ensure that you look and feel your best. We understand that going to a new doctor’s office can be a bit confusing, but rest assured we will explain what you can expect from us every step of the way. To allow us to serve you better on your first appointment, be sure to bring:

    • Identification
    • Insurance cards or forms provided to you by your insurance company
    • A list of any current medications you may be taking
    • Any recent x-rays if they are available

    Appointments

    Since surgery appointments are scheduled for at least one hour a minimum 72 hour notice is required, if you wish to change your appointment. If you have any further questions, please let us know.

    Payments

    Payments are due at the time of service. For your convenience, we are happy to accept Visa, MasterCard and American Express and will file all insurance on your behalf. Alternative payment methods are available upon inquiry.

    Insurance

    Berkeley Periodontics is a Delta Dental provider. We also deal with many different insurance companies and will be happy to file a dental claim, where applicable, with your insurer on your behalf.
